VISA APPLICATION FORM INSTRUCTIONS (簽證申請表須知)

  1. No column should be left blank. Incomplete/ vague entries shall not be accepted. Original supportive documents should be submitted along with the application form.
    請正確填寫表格，所有項目為必填，如有空缺則視作無效。如需提供說明或證明文件，請同申請表一同附上。
  2. Two (02) passport size photographs should be attached with the visa form.
    隨表請附兩張證件規格照片。
  3. Applicants could be asked to appear for interview if required.
    申請人或需接受面試。
  4. The application form has to be filled in English.
    申請表需用英文填寫。
  5. The application form must be signed.
